Physical Therapist for Adult and Geriatric Patients

About the Role:

We are seeking a dedicated and experienced Physical Therapist to join our team in providing care services for adult and geriatric patients in Senior Living Communities and Independent Living Communities.

Responsibilities:

1. Maintain a caseload of home-based physical therapy clients, making regular visits to assess and address their mobility needs.
2. Perform diagnostic tests and measurements, including assessments of joint mobility, transfer status, balance testing, and safety evaluations.
3. Develop and implement individualized care plans that may include manual therapeutic exercises, gait training, balance interventions, and other rehabilitative activities.
4. Continuously evaluate and revise client care plans as necessary, collaborating with nursing professionals on multidisciplinary care strategies.
5. Educate and instruct clients and their representatives on rehabilitative care techniques to promote independent living and overall well-being.
6. Diligently document all observations, interventions, and evaluations related to client care management using electronic medical records (EMRs).

Requirements:

• A current Pennsylvania Physical Therapist license
• A minimum of one year of recent experience as a Physical Therapist
• Graduation from a program accredited by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
• Ability to work independently and effectively manage time
• Strong interpersonal, organizational, and problem-solving skills
• Proficiency with electronic medical records (EMRs) and computer software
