Job Summary:
- Providing in-home physical therapy services to clients in need.
- Developing and implementing tailored care plans to address clients' specific needs.
- Collaborating with clients and their caregivers to ensure safe and effective rehabilitation.
- Maintaining accurate and up-to-date electronic medical records (EMR).
Requirements:
- A current PA Physical Therapist license.
- A minimum of one year of experience as a Physical Therapist in a home health setting.
- Graduation from an accredited PT program approved by the American Physical Therapy Association.
- Proven ability to work independently and collaboratively as part of a healthcare team.
- Competency in EMR software preferred.
