Responsibilities and Qualifications
- Make home visits to clients in designated geographic territories to conduct diagnostic tests and measurements, such as mobility and range of motion assessments.
- Develop and implement individualized care plans that address the specific needs and goals of each client.
- Educate clients, family members, and caregivers on proper techniques and strategies to enhance functional abilities and prevent complications.
- Accurately document observations, interventions, and evaluations in state-of-the-art electronic medical records.
The ideal candidate will hold a current North Carolina Physical Therapist license and have graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
