Key Responsibilities:
* Provide one-on-one physical therapy services to clients in their homes
* Conduct initial assessments and develop individualized care plans
* Implement evidence-based interventions to improve mobility, strength, and functional ability
* Collaborate with the interdisciplinary team to ensure comprehensive care
* Document all interactions and interventions in the client's electronic medical record
Requirements:
* Current licensure as a Physical Therapist in the state of Pennsylvania
* Graduation from an accredited physical therapy program
* Strong communication and interpersonal skills
* Ability to work independently and manage time effectively
* Prior experience with electronic medical records (EMR) preferred
