BAYADA Home Health Care is a leading provider of home-based care services. We are currently seeking an experienced Physical Therapist to join our team as a Senior Home Care Specialist in Schuylkill County, Pennsylvania.
Job Overview
The ideal candidate will possess a strong clinical background and excellent communication skills, with the ability to work independently and manage time effectively. They will be responsible for making home visits to clients, conducting diagnostic tests and assessments, and developing and implementing individualized care plans.
Key Responsibilities
- Conduct comprehensive assessments of clients' physical needs and develop personalized care plans to meet those needs.
- Perform diagnostic tests and measurements, such as mobility and balance assessments, to identify areas for improvement.
- Implement evidence-based interventions to promote wound repair, improve mobility, and enhance overall function.
- Work collaboratively with nursing staff to develop and revise multidisciplinary care plans.
- Educate and instruct clients, family members, or other caregivers on rehabilitative techniques and activities necessary to promote health and independence.
- Accurately document client interactions, assessments, and interventions in electronic medical records.
Qualifications
- A current Pennsylvania Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Excellent interpersonal, organizational, and problem-solving skills.
- Ability to work independently and manage time effectively.
