Job Description
We are seeking a highly skilled and compassionate Physical Therapist to join our team in providing high-quality physical therapy services to clients in their homes. This position requires a dedicated and detail-oriented individual who can provide evidence-based interventions to improve client function, mobility, and overall quality of life.
Responsibilities:
- Conduct thorough assessments and evaluations to identify client needs and goals.
- Develop and implement individualized care plans that address client-specific needs and objectives.
- Provide comprehensive physical therapy interventions to promote client independence, comfort, and overall well-being.
- Educate and instruct clients and their families on proper techniques for exercises, transfers, and other activities necessary for safe and effective care.
- Maintain accurate and timely documentation of client interactions, progress, and outcomes.
- Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care delivery.
Qualifications:
- Current licensure as a Physical Therapist in Pennsylvania.
- Minimum one year of experience as a Physical Therapist, preferably in home health care settings.
- Graduation from an accredited Physical Therapy program.
- Excellent communication, interpersonal, and problem-solving skills.
- Able to work independently and manage time effectively in a fast-paced environment.
