We are seeking an experienced Physical Therapist to join our team at BAYADA Home Health Care. As a member of our multidisciplinary healthcare team, you will be responsible for providing high-quality physical therapy services to clients in their homes.

About the Role

• Provide one-on-one physical therapy services to clients in their homes, developing individualized care plans that meet their unique needs and goals.
• Conduct home visits to assess clients' progress and make adjustments to their care plans as needed.
• Collaborate with other healthcare professionals, including nurses and therapists, to ensure seamless care coordination.
• Maintain accurate records of client interactions and progress, using state-of-the-art electronic medical records (EMRs).

Qualifications

• A current Pennsylvania Physical Therapist license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• At least 2 years of experience as a Physical Therapist, preferably in a home health care setting.
