We are seeking a skilled Physical Therapist to join our team and provide exceptional care to adult and geriatric patients in their homes.
The ideal candidate will have a current license as a Physical Therapist in Massachusetts and be able to make visits to clients in their homes, perform diagnostic tests and measurements, and develop and implement individualized care plans.
This role offers a unique opportunity to work with a multi-disciplinary healthcare team, providing skilled nursing and rehabilitative care to clients and affording them the opportunity to maintain their independence.
Key Qualifications
- Current license as a Physical Therapist in Massachusetts
- Able to make visits to clients in their homes
- Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing and safety assessments
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance and other interventions
