Job Description: The Home Health Physical Therapist will provide rehabilitative care to clients in their homes, making adjustments as needed to ensure optimal outcomes. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work independently and manage time effectively.
Responsibilities:
- Maintain accurate and up-to-date records of client care management and services provided
- Participate in multidisciplinary care planning, collaborating with nursing staff and other healthcare professionals as necessary
- Collaborate with clients and family members to educate them on rehabilitative care and activities
- Conduct regular assessments to evaluate client progress and adjust care plans accordingly
Qualifications:
- A current New Hampshire Physical Therapist license
- A minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Excellent communication and interpersonal skills
