Job Description
We are seeking a compassionate and skilled Registered Nurse to join our team. As a home health nurse, you will be responsible for providing one-on-one care to our clients in their homes. This is a rewarding opportunity to make a real difference in the lives of others.
Key Responsibilities:
- Assessing patients' needs and developing individualized care plans
- Providing medical treatments and therapies as prescribed by physicians
- Monitoring patients' conditions and reporting changes to Clinical or Client Services Manager
- Documenting all observations, interventions, and evaluations pertaining to client care management and services provided
Requirements
To be successful in this role, you must have:
- A current license as a Registered Nurse in PA
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience is strongly preferred but not required
