About the Job
This role requires a skilled Speech Language Pathologist who can work independently in patients' homes, assessing and addressing communication and swallowing difficulties. You will be responsible for developing and implementing treatment plans, educating patients and family members on care and activities, and documenting patient progress.
Responsibilities
- Assess and address communication and swallowing difficulties in adult and geriatric patients
- Develop and implement treatment plans based on evaluation results and physician's orders
- Continuously assess and revise the care plan as needed
- Document patient progress and communicate effectively with other healthcare professionals
