About the Job
You will be responsible for evaluating patients' physical and cognitive abilities, developing rehabilitation plans tailored to their needs, and educating and instructing patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Key Responsibilities:
- Evaluate patients' physical and cognitive abilities and develop rehabilitation plans tailored to their needs.
- Conduct thorough assessments of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
- Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
