Residential Healthcare Worker
As a skilled and compassionate Physical Therapist Assistant (PTA), you will play a vital role in providing personalized care and support to our clients in their homes. We are seeking a detail-oriented and dedicated individual to join our team in Avalon, New Jersey.
Responsibilities:
- Follow a designated care plan, ensuring the best possible outcomes for patients.
- Make regular home visits to clients, offering personalized care and attention.
- Administer medication, wound care, treatments, and procedures with precision and compassion.
- Monitor clients' conditions closely, reporting any changes or concerns to Clinical or Client Services Manager.
- Document observations, interventions, and evaluations accurately, utilizing state-of-the-art technology.
- Collaborate with other healthcare professionals to deliver comprehensive care and support.
- Manage client caseloads effectively, ensuring seamless transitions and continuity of care.
Requirements:
- A current license as a Registered Nurse in New Jersey.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is highly valued, but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and a patient-centered approach.
- Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
- Ability to travel to cases as assigned.
