Responsibilities
- Make home visits to clients in designated geographic territories to assess and treat physical impairments.
- Develop and implement individualized care plans to promote client independence and improve overall health outcomes.
- Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary to promote healthy living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements
- A current New Jersey Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong communication and interpersonal skills.
