About the Role
You will make outpatient visits to clients in designated senior living communities, providing personalized care and support to help them achieve their goals. Collaborate with our multidisciplinary team to develop and implement comprehensive care plans addressing client needs.
Responsibilities
- Conduct thorough assessments of client needs and preferences.
- Develop and implement individualized care plans, incorporating manual therapeutic exercises, gait training, balance, and other interventions.
- Continuously monitor client progress and adjust care plans as needed.
Requirements
- A current CO Physical Therapist license.
- Experience working in outpatient settings, although not required.
- Education from an accredited Physical Therapy program.
- Excellent communication and interpersonal skills.
