As a skilled Home Health Physical Therapist, you will be part of our multidisciplinary healthcare team, providing outpatient services to clients in the Fort Collins area.
Job Description
Our ideal candidate will have a current CO Physical Therapist license, graduation from a program approved by the American Physical Therapy Association, and strong interpersonal and organizational skills.
You will be responsible for making outpatient visits to clients, supervising Physical Therapy Assistants, attending patient care conferences, and referring clients to other disciplines as needed.
