Provide rehabilitative care and skilled nursing to clients in their homes throughout Orleans County and surrounding areas
Conduct thorough assessments of clients' needs and develop personalized care plans
Collaborate with other healthcare professionals to ensure seamless care coordination
Communicate effectively with clients, family members, and caregivers to ensure understanding and adherence to care plans
Educate clients and caregivers on proper techniques for safe and effective rehabilitation
Document all interactions and interventions accurately and thoroughly using state-of-the-art technology
Requirements
Current licensure as a Physical Therapist in Vermont
Minimum one year of experience working as a Physical Therapist
Graduate of a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
Excellent communication and interpersonal skills
Ability to work independently and manage time effectively
Proficiency in electronic medical records (EMR)
