About the Role:
As a Home Health Physical Therapist at BAYADA, you will play a critical role in improving the lives of our clients. You will work independently, assessing clients' physical abilities and developing personalized care plans. You will also conduct diagnostic tests and measurements, implement evidence-based interventions, and educate clients and family members on rehabilitative care techniques.
Responsibilities:
Visit clients in their homes to assess their physical abilities and develop personalized care plans;
Conduct diagnostic tests and measurements to identify areas for improvement;
Implement evidence-based interventions to promote wound repair, improve mobility, and enhance overall well-being;
Continuously assess and revise care plans to ensure optimal outcomes;
Educate clients and family members on rehabilitative care techniques;
Accurately document observations, interventions, and evaluations;
Requirements:
A current NJ Physical Therapist - PT license;
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association;
Ability to work independently and manage time effectively;
Strong interpersonal, organizational, and problem-solving skills;
Solid computer skills, preferably with experience using electronic medical records;
