Job Summary:
We are seeking a highly skilled Registered Nurse to join our team as a Home Health Practitioner. In this role, you will be responsible for providing comprehensive nursing care to clients in their homes, working collaboratively with interdisciplinary teams to ensure seamless coordination of care.
Responsibilities:
- Develop and implement personalized care plans tailored to each client's unique needs and goals.
- Conduct thorough assessments and evaluations to identify areas for improvement and track client progress.
- Administer medications, perform wound care, and provide treatments as prescribed by healthcare professionals.

Qualifications:
- A current license as a Registered Nurse in NJ.
