We are recruiting an accomplished Speech Language Pathologist to be part of our interdisciplinary team, delivering rehabilitative care and skilled nursing services directly to clients' homes.
Position Overview:
This role focuses on assessing, treating, and rehabilitating individuals with communication and swallowing impairments. As a key member of our team, you will work closely with physicians and other healthcare professionals to create customized treatment plans tailored to each client's unique needs.
Responsibilities:
- Evaluate and diagnose clients with speech, language, and swallowing disorders.
- Develop and implement individualized treatment plans to address client goals and objectives.
- Accurately document client interactions, treatments, and progress in electronic medical records.
- Work collaboratively with healthcare providers to ensure coordinated care delivery.
- Provide patient education and support to enhance independence and quality of life.
Qualifications:
- Holding a current licensure as a Speech Language Pathologist in New Jersey.
- Possessing a certificate of clinical competence from the American Speech-Language-Hearing Association or equivalent expertise.
