Duties and Expectations
- Make home visits to assess and diagnose clients with speech, language, communication, or swallowing difficulties.
- Collaborate with physicians and other healthcare professionals to develop individualized treatment plans.
- Educate patients and their families on rehabilitative care and activities necessary for independent living.
- Maintain accurate and detailed records of patient care management and services provided using electronic medical records.
Requirements
- A current license as a Speech Language Pathologist in New Jersey.
- Certificate of clinical competence in speech from the American Speech and Hearing Association or equivalent education requirements.
- Excellent communication and interpersonal skills.
