Job Summary
We are seeking a skilled and compassionate Home Health Speech Language Pathologist to join our team. In this role, you will provide high-quality speech therapy services to clients in their homes, working closely with our interdisciplinary team to develop and implement comprehensive care plans.
Key Responsibilities
- Assess and diagnose speech and language disorders in clients
- Develop and implement individualized treatment plans based on client goals and needs
- Provide one-on-one therapy sessions to clients in their homes
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination
- Maintain accurate and timely documentation of client progress and services provided
Requirements
- Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association (ASHA)
- Current license as a Speech Language Pathologist in PA
- Solid computer skills, including experience with electronic medical records (EMR)
- Excellent communication and interpersonal skills
