Bayada Home Health Care is seeking a Support Services Coordinator to join our team in Pittsburgh, PA. In this role, you will have the opportunity to provide critical support to our clients and their families by coordinating and delivering high-quality client care services.
About the Job
The Habilitation Manager will work closely with the Clinical Manager to develop and implement effective client care plans. This will involve participating in Person-Centered Planning processes, collaborating with the client, family, and other stakeholders, and carrying out supervision of Habilitation Technicians.
Responsibilities
- Coordinate and deliver high-quality client care services through collaboration with the Clinical Manager and other stakeholders.
- Participate in Person-Centered Planning processes to identify individual client goals and needs.
- Work closely with the client, family, Case Managers, Hab Techs, and other teams to implement and evaluate client care plans.
- Maintain ongoing communication with Case Managers/Care Coordinators to ensure seamless client care transitions.
Requirements
- Bachelor's degree required.
- Must be a Qualified Professional with minimum years of post-degree experience working with individuals with intellectual/developmental disabilities.
- Prior supervisory experience is a plus.
- Demonstrated record of successfully taking on increased responsibility.
- Ambition to grow and advance beyond current position.
- Excellent communication and interpersonal skills.
