About the Job:
\BAYADA Home Health Care is seeking a skilled and compassionate Occupational Therapist to join our team. In this role, you will work closely with our multidisciplinary healthcare team to provide rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
\About the Role:

- Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or in group homes for adults with intellectual disabilities. \
- Work closely with your home health therapy partners to ensure seamless transitions from home health to outpatient services and achieve client-centered goals in their living environment. \
- Collaborate with clients, families, and caregivers to educate them on care plans and promote a team approach to maximizing functional independence. \
- Develop and implement individualized treatment plans that cater to the unique needs of each client, utilizing an outpatient EMR platform to document progress and outcomes. \
- The ideal candidate will have a current PA Occupational Therapist license, prior home care experience, a minimum of one year of recent work experience as an Occupational Therapist, and solid computer skills. \
