About the Position
You will be responsible for assessing patients' functional capabilities, developing evidence-based intervention plans, and collaborating with other healthcare professionals to ensure that care is coordinated, effective, and responsive to patients' changing needs.
Key Accountabilities
- Evaluate patients' physical, emotional, and cognitive abilities to identify areas for improvement and develop targeted intervention plans.
- Implement and adapt intervention plans to address emerging needs, ensuring that care remains person-centered and responsive to patients' changing circumstances.
- Collaborate with other healthcare professionals to coordinate care delivery, share best practices, and optimize patient outcomes.
- Provide education and support to patients and their families to empower them to take an active role in managing their health and promoting long-term wellness.
You should have a current North Carolina Occupational Therapist license, a minimum of one year of recent work experience as an Occupational Therapist, and graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
