As a Home Health Occupational Therapist, you will play a vital role in our team, working closely with clients, families, and other healthcare professionals to deliver personalized care and rehabilitation plans. You will have the opportunity to make a real difference in the lives of our clients, helping them to achieve their goals and regain independence in their daily lives.
Key Responsibilities
- Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop individualized intervention plans to achieve favorable outcomes, taking into account patients' goals, needs, and preferences.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote independent living.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
Requirements
- A current Vermont Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
