What are the responsibilities and job description for the Therapy Specialist in Home Setting position at BAYADA Home Health Care?
Key Responsibilities
- Making home visits to assess client needs and develop personalized care plans.
- Performing diagnostic tests and measurements, such as mobility assessments, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance and other interventions.
- Educating and instructing clients, family members or other representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
You will also be responsible for accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided using a touch pad tablet.
