The Care Coordination Assistant Responsibilities Include:
- Entering new referrals and updates all existing patient information in corporate IS systems including demographics, insurance, and benefit information.
- Verifying insurance and benefits for all Medicare, Managed Care and Private Fee for Service Plans. Securing authorization for services provided by BayCare HomeCare from the payers.
- Selecting the correct payer codes to provide accurate intake information.
- Understanding medical criteria for securing all required information from the referral source.
- Entering the appropriate financial information including benefits, deductibles, co-pays, coinsurance, out-of-pocket, another related costs into the EMR system.
- Timely requesting and communicating of authorizations to payer and documentation in EMR.
- The Care Coordination Assistant will work under the direction of the Care Transition Coordinator or RN Case Manager to establish and maintain a positive working relationship with patients, payors and other customers.
Position details:
- Location: Sarasota Memoria Hospital/Venice Location - Venice, FL
- Status: Part time, 16 hours per week, but flexibility for possible additional hours
- Schedule: 8:00 AM - 5:00 PM
- Weekend requirement: Every or Every other, flexible
- On Call: No
Education:
- Required High School Graduate or Equivalent GED
- Preferred Technical Diploma – Medical Specialization
Experience:
- Required 1 year Customer Service
- Required 1 year Insurance Verification
- Required 1 year Medical Related field
