- Status: Full-time, salary
- Shift: 8:00 AM - 4:30 PM
- Occasional weekends

The Clinical Nurse Navigator serves as the coordinator for the care of orthopedic patients
- Responsible to provide support, education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience.
- Serves as the coordinator for the care of patients in their specific specialties.
- Responsible for facilitating integrated care through communication with strategic members of the interdisciplinary team, collaborating to provide staff education, and works closely with patients, families and members of the health care team to optimize patient satisfaction and achieve optimal outcomes.
- Coordinate efforts between physicians, healthcare administration, family, community resources and staff while tracking the progress of the patient throughout their continuum of care.
- Promotes evidence-based clinical care that exemplifies the pursuit of clinical excellence.
Requirements:
- RN (Registered Nurse) License
- Required Bachelor Degree in Nursing
- 2 years of acute care experience
- Required Certification in Related Specialty must be obtained within the first 6 months if not current.
- A background working in Orthopedics is preferred
