Facility: BayCare Health System, Behavioral Health Admin-DBH
Location: Mease Dunedin
Status: Full Time, Exempt: No
Shift Hours: 8:00am-4:30pm
Weekend Work: None
On Call: No
Discharge Planner Pool - Responsibilities:
- Provide assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
- Possesses a clear understanding of discharge and transition planning and linkage with community resources.
- Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
- Provides support as active member of the treatment team.
- Travel may be required.
- Will perform other duties as assigned by supervisor.
Experience:
- Required: 1 year of Behavioral Health and 2 years of Child and Adolescent (SIPP Only)
- Preferred: 6 months in Acute Care
Education:
- Required: Bachelor's in counseling
- Or Bachelors in: Health Education, Social Work, Human Services, Psychology or Nursing
- Preferred: Masters in human services
Credentials:
- Required - Basic Life Support (Basic Life Support)
