The Social Services Specialist responsibilities include:
- Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
- Provides coordination, referral and interdisciplinary discharge planning
- They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
- Possesses a clear understanding of discharge and transition planning and linkage with community resources
- Demonstrates skills in planning, organizing, and managing multiple functions
Position details:
- Location: Dunedin Hospital - Dunedin, FL
- Status: Part Time, 16 hours per week
- Schedule: Saturday & Sunday 10:00 AM - 6:30 PM
- Weekend Requirement: Every
- On Call: No
Certifications and Licensures:
- Preferred ACM (Case Management) or
- Preferred CCM (Case Manager)
Education:
- Required Bachelors in Social Work, Sociology, or Related Field
Experience:
- Required 6 months Internship or
- Required 6 months Social Work in an acute care setting
