General Summary:
As a member of Care Management within the Emergency Department participates in the planning, facilitation, coordination of care for the Emergency Department behavioral health and/or substance abuse patient.
Responsibilities:
1. In coordination with Emergency Department care delivery team, conducts assessment of patient needs.
2. To optimize safe and effective care delivery, facilitates in-patient and/or out-patient placement discussions with organizational, community, and state partners/stakeholders.
3. Completes daily rounding on all behavioral and substance abuse identified patients to 1. Investigate and facilitate options/need for patient transfer, 2. Ensure patient and staff well-being, 3. Other.
4. Interacts with organization, community, and state resources to evaluate and plan for crisis care of vulnerable patient populations.
5. Acts as the primary Emergency Department resource for patient referral and placement – provides real-time process intervention, develops and maintains ED staff and physician resource database targeting behavioral health and patients experiencing substance abuse or opioid use disorder complaints.
6. Works as a member of Care Management within the Emergency Department care delivery team. Functions as a member of the Care Management social work team.
7. Accurately and efficiently document patient interactions and process interventions.
8. Collaborates with Chair, Senior Director, Social Services Manager and Emergency Department medical and nursing staff on evaluating overall activities and commitment to high reliability relating to the care of emergency medicine behavioral health patients.
9. Collaborates with organizational leadership on development and validation of best practices.
10. Provides social work services and discharge planning to patient population across the continuum of illness. Promotes effective utilization and seamless transition to acute and post-acute care. Collaborates with patients, families, healthcare team members, and community agencies to develop and implement plans to address identified needs.
11. Assists in the implementation, coordination, assessment, evaluation and development of Bayhealth’s behavioral health programs within the Department of Emergency Medicine.
12. Applies, teaches, and skillfully uses techniques that enhance the care of patients suffering from psychiatric illnesses.
13. Identifies problematic issues related to department care defects, determines reasons and actions to resolve. Communicates suggested corrective action plan to interested parties and follows up to determine current status.

Required Education, Credential(s) and Experience:
- Education: Master Degree

Required: One (1) year in a healthcare setting and/or mental health counseling.
Preferred: One– 2 years behavioral health crisis intervention setting and /or mental health Information and referral services experience.
