General Summary:
Under the general supervision of Pharmacy Leadership, the Diabetes Educator is responsible for providing educational services related to Diabetes Care to patients and Healthcare Providers. This position supports compliance with American Diabetes Association Education Recognition Program guidelines.
Responsibilities:
1. Provides Diabetes Self-Management Education and Support (DSMES) for participants aged 18 and older in compliance with ADA Education Recognition Program (ERP) guidelines, including intensive insulin therapy and management of diabetes during pregnancy and diabetes resulting from treatment of another disease.
2. Collaborates with other members of the health care team (nursing, medical, nutrition and pharmacy) to assess, plan, identify priorities and realistic outcomes.
3. Utilizes comprehensive assessment skills in the evaluation of learning and lifestyle modification needs and abilities as well as barriers to care in adult patients with diabetes.
4. Assesses educational needs of participants/families and provides culturally sensitive participant/family interventions through individual and group sessions, as indicated. Uses non-technical (patient-friendly) language when educating patients/families. The education plan is participant-centric and follows EBP guidelines per the current year’s National Standards for Diabetes Self-Management Education and Support - NSDSMES.
5. Documents assessment, education plan, intervention, outcomes in the patient’s Electronic Health Record (EHR).
6. Identifies and implements interventions that are participants specific and assists patient in obtaining their self-management goals.
7. Utilizes problem solving techniques and collaborative care, identifies and addresses psychosocial issues, behavior change, and strategies for patient to achieve successful diabetes self-management.
8. Identifies ongoing support and resources to assist participant in achieving their diabetes self-management goals. Maximizes participant outcomes.
9. Evaluates effectiveness and cost efficiency of Diabetes Self-Management Education and Support.
10. Assures compliance with regulatory and accreditation standards: a. Documents participant assessments and education as required by Joint Commission, ADA Recognition, and Medicare. b. Promotes evidence based practice through staff education and leadership in developing, implementing and evaluating process standards, such as plans of care, protocols and guidelines.
11. Participates in community events for community education and program marketing. a. Exceptional: participates in 4 community events b. Superior: participates in 3 community events c. Meets: participates in 2 community events d. Below: participates in 1 or less community events
12. Participates in the Diabetes Advisory Group via planning activities, preparation, meeting attendance and participation.
13. Provides administrative support to the Program Coordinator for the adherence to the National Standards for DSMES.
14. All other duties as assigned, within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience:
Education: Must be a graduate of an accredited nursing program/university with a Bachelor Degree in Nursing BSN or higher.-
Credential(s): Registered Nurse RN -
Experience: Requires 2 or more years of professional practice as an RN, RD, or Pharmacist.
