Job Overview
We are looking for a highly skilled Patient Outcomes Manager to join our team at Bayhealth. As a Patient Outcomes Manager, you will be responsible for delivering high-quality patient care while maintaining a commitment to quality and safety.
The ideal candidate will have experience in relevant specialties and hold a nursing diploma from an accredited program or university. Basic Life Support certification is mandatory, with Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certification preferred.
This position requires a strong understanding of the nursing process and ability to develop a patient-centered plan of care based on individualized needs. The successful candidate will also be able to maintain confidentiality of the patient medical record, counsel and educate patients and their caregivers on treatment plans, and administer medication following the five rights of medication administration.
