The Oncology Clinical Pharmacy Specialist plays a critical role in ensuring the delivery of high-quality care to cancer patients. This position involves working closely with healthcare teams to optimize treatment plans, educate patients and healthcare professionals, and promote evidence-based practice standards.
Responsibilities:
- Collaborate with oncologists to develop and implement evidence-based practice standards for supportive care measures.
- Develop and deliver educational programs for pharmacy staff, nursing staff, and other healthcare professionals on oncology-related topics.
- Serve as a subject matter expert in oncology, providing drug information and consulting on Electronic Health Record care plans.
- Collaborate with healthcare teams to optimize treatment plans according to established national evidence-based guidelines and policy.
- Provide patient-specific medication education using the teach-back method for new oral oncolytics and intravenous chemotherapy.
- Review oncology care plans within the Electronic Health Record, including new plans and updates to existing plans.
- Identify and evaluate medication-related problems based on clinical presentation, history, and laboratory data.
- Serve as a preceptor to pharmacy residents and students.
- Participate in Oncology Service Line meetings to improve safety, efficacy, and quality of cancer care.
- Promote rational and cost-effective drug therapy, focusing on cost avoidance and savings within the Oncology Service Line.
