Location:
Inners Creek Nursing and Rehab

Contract Duration: 9 Weeks
Shift: 3:00 PM - 11:30 PM (Evening)
EOW (Every Other Weekend) Required
Pay Rates:
- Local: $30/hr on W2
- Travel: $34/hr ($9/hr on W2 $1000/weekly per diem)
Requirements:
- Experience: 2 years of LPN LTC experience
- License: Active LPN License
- Certifications: BLS & COVID Card
- References: Two supervisor references with:
- Full Name
- Contact Number
- Title
- Facility Name
- Last 4 Digits of SSN
