About the Role:
You will work closely with doctors, nurses, and other team members to evaluate patient progress and develop an individualized plan of care. Using evidence-based practices and a trauma-informed approach, you'll help patients develop individualized coping techniques and symptom management skills.
Key Responsibilities:
- Provide individual and group counseling services
- Conduct initial assessment and evaluation to determine eligibility for services
- Complete bio-psychosocial assessment
- Establish individualized treatment plan with each patient
- Respond to patient grievances and complaints
- Develop aftercare and discharge plans
Requirements:
To be successful in this role, you must be a Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC). Additionally, you should demonstrate a positive, non-judgmental, and accepting attitude toward our patient population.
