Recovery Case Manager Role
The Recovery Case Manager is a key member of the team who works closely with clients to ensure their successful recovery. With compassion and integrity, they plan and implement aftercare plans that meet the unique needs of each client.
Responsibilities:
- Hold clients accountable for their actions in the community and completion of work tasks
- Facilitate timely documentation and monitoring of client contracts
- Monitor client compliance with phone policy, buddy relations, and other program requirements
- Communicate effectively with clients, family members, and referral sources through written, electronic, and oral means
- Make sound decisions and find alternative solutions when needed
- Conduct weekly calls with clients' family members to discuss progress, behavior, and growth opportunities
- Schedule face-to-face meetings with family members and loved ones as needed
- Execute step-downs and discharges according to established protocols
- Upload and delegate current ROIs and aftercare plans into the Electronic Medical Record (EMR)
- Attend regular team meetings, including Morning Meeting, RCM Meeting, Supervision, Safety Squad, and Quality Assurance
- Engage with clients beyond individual case loads to promote overall well-being
Qualifications:
- High school diploma or equivalent required
- Extensive experience in Mental Health and Substance Use Disorder industry
- Strong time management skills and ability to work on multiple projects simultaneously
- Effective coordination with all departments and oversight of clients' stay at Granite Medical Treatment Center
- Creation of realistic after-care plans that meet clients' needs
- Vetting of client visitors and phone calls
- Meet and maintain pre-employment requirements, including reference checks, background screening, drug testing, DMV check, insurance verification, and license/credential verifications
