Schedule: Full-Time; 40-hours per week

Job Summary:
Under the general supervision of the Assistant Director of Behavioral Health, the Case Manager provides case management services to assist eligible individuals to access medical, social, pre-vocational and other needed community services. Services are directed towards achieving the individuals' desired results and personal milestones.
Duties/Responsibilities:

Collect and evaluate psychological, historical, and environmental data from clients, appropriate sources, and documentation to assess:

The client’s current psychological status and environment.

The client’s personal resources and coping abilities.
-
The client’s capacity to access and utilize additional resources effectively.
-
Inform clients and stakeholders about privacy rights, confidentiality policies, grievance procedures, mandatory reporting requirements, and program rules and services.
-
Collaborate with staff and clients to establish service priorities and intervention strategies.
-
Identify and document client-focused goals with input and agreement from the client.
-
Track and document client adherence to service plans and progress toward goals.
-
Modify service plans as needed in collaboration with clients to address changes or challenges.
-
Advocate or act as a liaison with other service providers, including medical staff, social services, and housing agencies.

Build and maintain knowledge of community resources relevant to the client population.

Assist clients in accessing community resources such as housing, financial assistance, food, clothing, and legal services.

Arrange pre-placement visits, placement services, and negotiate contracts when necessary.

Promote and assist in developing life skills, including:
-
Personal care and property maintenance.
-
Interpersonal relationship management.
-
Rent and bill payments.
-
Budgeting and money management.
-
Provide direct assistance in performing routine daily tasks as needed.

Offer substitute payee services when appropriate.

Encourage the use of primary medical and dental care services.

Assist clients in scheduling and attending healthcare appointments.

Promote healthy lifestyle choices aligned with the client’s goals and needs.

Educate clients, families, and relevant stakeholders on:

The client’s status and individual service plan.

Availability and use of community resources.

Progress toward service plan objectives.

Provide tailored support to achieve service plan outcomes.

Maintain compliance with departmental policies, procedures, and performance standards.

Performs other related duties as assigned.

May be assigned cross departmental responsibilities.

Education and Experience:

Possession of a Bachelor’s Degree in a Social or Behavioral Science from an accredited college or university and one (1) year of case management experience OR possession of an Associate Degree (AA) or its equivalent in a social or behavioral science related field plus three (3) years of case management experience.

Knowledge of case management principles and practices, skills in record keeping, money management, and problem resolution.

Knowledge of plan development, placement services, and linkage and consultation principles.
-
Experience in working with the mentally ill in an urban area desirable.

Specific vision abilities required by this job include close vision, peripheral vision, and the ability to adjust focus.
