Director of Social Services

Job Summary
The Director of Social Services will lead and oversee the development and implementation of social service programs aimed at improving the quality of life for individual and, particularly those with developmental disabilities. This role requires a strategic thinker with strong leadership skills.Assist in planning, developing, organizing, implementing, evaluating, supervising and directing the social services programs and activities, consult with and coordinate with social services with administration, medical and nursing services, consultants and other related departments and assist in implementing changes as required; obtain residents' social histories and evaluate social and family information, assist in determining plans for social treatment and interpreting residents' social, social, psychological and emotional needs to the medical staff and other resident care team members, participate in implementation of social care plans & resident assessments, provide information to residents and their families concerning available support agencies and financial assistance programs to meet the needs of the residents.

Duties

• C omplete MDSs based on Calendar given by MDS Director
• BIMS Score on all new admits/Quarterly for LTC Residents
• DNRs-Initiated on Admission if requested. Ensure follow-through with nursing department
• Responsible for referrals for Ancillary Services (Audiology, Dental, Podiatry)
• Schedule/attend LTC Care Plans based on MDs Calendar given by MDS Director
• Intervene with family/residents concerns
• Handle all grievances, when grievance is completed by appropriate department give completed copy to Administrator. Keep log of grievances (State asks for this - will also be needed for QA meetings)
• Keep log of ancillary referrals and Grievances to present during Q&A

Qualifications

• LSW Licensure plus 2 years experience i Social Services
• Proven experience in program development and management within social services or behavioral health settings.
• Strong supervisory skills with a track record of managing teams effectively.
• Excellent relationship management abilities, capable of engaging diverse situations
• Knowledge of budgeting processes and financial management related to social service programs.
• Experience working with individuals and maintaining resident confidentiality
• Exceptional organizational skills with attention to detail; ability to manage multiple projects simultaneously.
• Strong communication skills, both verbal and written, are essential for effective advocacy and reporting.

Pay: $18.00 - $25.00 per hour
