What You’ll Do:
- Provide comprehensive case management and coordinate care plans for home-based clients.
- Conduct client assessments, monitoring, and evaluations to ensure optimal care.
- Collaborate with physicians, therapists, and caregivers to support client recovery.
- Educate clients and families on disease management, medications, and care plans.
- Ensure compliance with medical standards and maintain accurate client records.
What We’re Looking For:
- Registered Nurse (RN) license in Minnesota (required).
- Experience in home health, case management, or clinical care (preferred but not required).
- Strong communication and organizational skills with a client-centered mindset.
- Ability to work independently while being part of a supportive team.
