General Summary : T his position is responsible for planning, developing, organizing, implementing and directing person-centered care programs and events for residents with Alzheimer’s and other related types of dementia.
Essential Job Duties :
- Conduct assessments, and resident evaluations, and develop care plans to meet the needs and preferences of the individual residents.
- Keep residents, staff, and families informed of scheduled activities and events.
- Develop monthly calendars that encompass all 7 dimensions of wellness and maintain inventory for all programming supplies.
- The ideal individual for this position will have experience or knowledge in dementia programs to meet the individual needs and interests of residents.
- Coordinate scheduling, training, and education of care staff regarding memory care programs / activities.
- Prepare monthly reports for QAPI meetings.
- Develop programs to overcome specific disabilities while creating a desirable social environment. Keep residents, staff, and families informed of scheduled activities and events.
- Assists with activities such as exercise, intellectual programs, games, arts and crafts, relaxation and spiritual programs, and social events to assist residents in developing interpersonal relationships to socialize effectively and to develop the confidence needed to participate in group activities.
- Seeks opportunities to meet residents’ psycho-social needs through both care-planned and spontaneous actions.
- Seeks opportunities to support the social needs of all members of the household through support, friendship, and kindness.
- Seeks opportunities to share personal interests and talents with members of the household through both planned and spontaneous activities.
- Help with visits and assist the staff with serving dinner.
- Attends in-service training and education sessions, as assigned; completes Relias training on time every month.
- Must be familiar with emergency procedures and possess the ability to assist with evacuations.
- Performs other duties and responsibilities as assigned.
