Supervises : Case Managers, Educator, and Community Health Workers.
Job Summary : The Program Supervisor is responsible for directly supervising the Lead Case Manager, Case Managers, and Community Health Workers through quality assurance and improvement functions such as home / outreach observation visits, weekly productivity meetings, and chart audits.
Essential Functions :
1. The supervisor has oversight of the lead case manager and community health workers' services to clients, including, but not limited to, intake assessments, state benefit assessments / applications, goal setting, case plan development, home visits, progress monitoring via standardized testing tools, client advocacy, and referrals, while maintaining compliance with established state and federal standards and requirements.
2. Coordination of supplemental programs and projects such as collaborative improvement, local community coalition involvement and innovation networks including community action networks.
3. Participate in conference calls with partner agencies as assigned by the program director.
4. Ensure appropriate referrals from lead case manager to case managers are based on client case histories and needs.
5. Conduct group health education classes, as needed, including specified training as required by supplemental programs, which include submission of required evaluation forms and data entry into our database.
6. Utilize productivity tools to monitor the productivity of the lead case manager, case managers, and community health workers and provide constructive feedback.
7. Implement BCFS safety protocols, at all times.
8. Work evenings, weekends, and holidays as needed or requested by the program director.
9. Participate in scheduled meetings and team decisions and operations.
10. Participate in workshops, seminars, education programs, and activities promoting professional development.
11. Maintain BCFS HHS's professional and ethical standards of conduct outlined in the employee handbook, including demonstrating respect for agency staff, children, and community members and complying with the required dress code, at all times.
Measurable Deliverables :
1. Meet with lead case managers, educators, and community health workers every week to review their weekly progress reports and provide guidance as required.
2. Review / approve chart audits performed by the lead case manager monthly or as requested by the program director.
3. Conduct monthly reviews to ensure case managers conduct the required home visits and client contact to guide the client's risk level.
4. Conduct weekly and monthly reviews to ensure CHW conducts two prenatal intakes a week and seven education intakes per month.
5. Conduct weekly and monthly reviews to ensure the educator conducts two health education classes weekly and enrolls seven new education participants monthly.
6. Document client visits, interactions, outcomes, and education provided to participants in Salesforce within two business days.
7. Complete accurate documentation by agency policies and procedures, contract and regulatory requirements, standards, and program timeline requirements.
8. Review Salesforce reports for compliance with program requirements.
9. Meet all pre-determined reporting deadlines that program, state, and federal partners require.
10. Meet all federal and state regulatory guidelines and standards for this position.
11. Collaborate with the Program Supervisor to provide group-based education to at least 250 unduplicated participants annually.
12. Collaborate with community health workers and case managers to ensure 450 case-managed / care coordination clients are served annually.
13. Demonstrate the ability to :
a. Respond sensitively and competently to the service population's cultural and socio-economic characteristics.
b. Communicate effectively in writing and verbally in English.
c. Work in a fast-paced environment.
d. Maintain computer literacy required to meet the responsibilities of the position.
e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
f. Travel as needed.
14. Other job duties as assigned per grant requirements.
Requirements :
1. Maintain certificates required for the position including but not limited to First Aid and CPR.
2. Valid Texas Driver's license with a clear driving record and reliable transportation.
3. Pass a pre-employment drug screen and random drug screens.
4. Provide proof or work eligibility status upon request.
5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
