What are the responsibilities and job description for the Social Services Assistant position at Beachside Center for Rehabilitation and Nursing?
General purpose: The primary purpose of your job position is to assist the Social Worker/Discharge Planner in the Social Service Department.
I. Education & Qualifications:
1. Experience in long term care facilities.
2. Computer skills
3. Florida CNA license
II. Physical & Health Requirements:
1. TB skin test and a physical prior to employment, and annually thereafter. Must show evidence of good health and be free of communicable diseases.
2. Must be able to read print with visual aides, if required.
3. Good grooming, with a neat and clean appearance.
4. Pass a 10 panel drug test (Drug free work place)
5. Must be able to respond to the paging system in the appropriate manner.
6. Able to sit for time necessary to complete paperwork duties required.
7. Able to work flexible hours, as needed.
8. Ability to ambulate throughout facility as needed to complete all job responsibilities.
9. Able to lift, carry, push and pull 30-35 lbs.
III. Working Conditions:
1. Sitting, standing, bending, lifting and moving intermittently during work hours.
2. Subject to frequent interruptions.
3. Attend and participate in continuing education programs
4. Hours as assigned by the Director of Social Services with flexibility of hours and days as needed.
5. Also works as a CNA when needed.
IV. Responsibilities and Duties:
1. Manage the “non-coverage” insurance notices.
2. Enter discharge instruction data and provide nursing stations with discharge notice slips.
3. Collaborate with rehab, resident and /or family regarding need for home health services and equipment. Contact agencies and provide documents needed to establish services.
4. Assist with gathering documents/records pertaining to insurance appeal notices, nursing home and ALF transfers.
5. Assist with face sheet revisions.
6. Assist the social service department with addressing, resident/family questions or concerns.
7. Assist with making ancillary (i.e. dental hearing aides, vision) appointments, gathering records/documents needed for the appointment and making transportation arrangements.
8. Collect demographic information from new residents and complete interview form for admission to collect social history.
9. Assist in obtaining resources from community social, health and welfare agencies to meet needs of resident.
10. Maintain the confidentiality of all resident care information.
11. Knowledgeable of residents rights, including the right of refusal.
12. Assist the social service director as needed with designated tasks throughout the facility which include but not limited to filing and shredding.
13. All other duties as assigned by Director of Social Services.
