Summary
Reports to: Patient Care Services Manager
Role Summary: The Market-Facing Patient Care Coordinator is responsible for enhancing patient experience by providing exceptional care and support. This role involves direct interaction with patients, ensuring their needs are met efficiently and compassionately, while also communicating effectively with the healthcare team and external stakeholders.
Key Responsibilities:
Patient Care:
- Assist patients with activities of daily living as delegated by the RN.
- Monitor and document patient information, including vital signs, intake/output, and any changes.
- Ensure a safe and comfortable environment for patients, attending to hygiene, grooming, mobility, and dietary needs.
Communication:
- Maintain clear and prompt communication with nurses, physicians, and other healthcare team members.
- Provide accurate shift reports and relay pertinent information in a timely manner.
- Respond promptly to patient, family, and staff needs, including emergency situations.
Professional Practice:
- Uphold patient privacy and dignity, adhering to ethical standards and hospital policies.
- Take ownership and pride in delivering high-quality care.
- Act as a role model and mentor for new associates.
Supply Management:
- Monitor stock supplies and report shortages.
- Keep patient rooms and department areas neat and orderly.
Leadership:
- Accept accountability and responsibility for addressing issues and conflicts.
- Follow up with the unit director as needed.
- Participate in councils, committees, and process improvement activities.
Self-Care:
- Balance workload and schedule breaks to manage stress.
- Utilize vacation days to renew and refresh.
Requirements:
Education:
- High school diploma or equivalent preferred.
Experience:
- Previous patient care experience preferred.
- Nurse Assistant certification preferred.
Skills:
- Basic computer knowledge and skills.
- Familiarity with hospital equipment and procedures.
- Ability to perform vital signs correctly.
- Strong team player with effective communication skills.
Certifications:
- Must obtain Basic Cardiac Life Support (BCLS/CPR) within 3 months of hire and maintain certification.
Working Conditions:
- Moderate stress involved, especially during high census or acuity periods.
- Potential exposure to infectious diseases and combative patients.
