Physical Therapist - Home Health

Experience :

Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

License :

Must have valid PT license

Certifications : CPR, BLS

Must-Have :

  • Proficiency in HomeCare HomeBase (HCHB) and Oasis
  • Reliable transportation with current driver's license and liability insurance
  • Open to traveling up to 75 miles from home clinic
  • Comfortable with 30 points per week productivity expectations

Job Description :

  • Provide, coordinate, and direct home nursing care based on physician's orders and agency policies
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses
  • Conduct home visits and periodic office meetings
  • Average 5-8 patients per day with various cases such as Start of Care, Wound Care, and Follow up Visits
  • Complete all documentation within 24 hours of the appointment
  • No guaranteed hours, must hit weekly productivity points
  • Ability to float to nearby clinics if needed for patient coverage
  • Proficiency in HomeCare HomeBase and Oasis EMR systems
  • Conduct all visits and activities according to agency standards and guidelines
