JOB SUMMARY
Due to an expansion in funding, Beacon is seeking to fill four full-time (40 hours/week) Case Manager position to support our work with individuals and families in our Permanent Supportive Housing program.
The Case Manager's primary responsibility will be to work with residents in the Crawford Homes Program to provide crisis resolution and flexible, intensive services related to client needs and goals in service of stabilizing and retaining housing. Must maintain accurate documentation and recordkeeping in online database in keeping with grant stipulations. Will involve occasional transportation to appointments.

Preferred qualifications: Bachelor’s degree in social services. 2 years working directly with individuals who have a serious mental illness and/or co-occurring substance use disorder. Case management experience.
Required: College degree. One year of experience working directly with vulnerable populations such as individuals experiencing homelessness or adults with disabilities. Excellent interpersonal and communication skills. Intermediate proficiency in computer skills. Must have a driver’s license and access to a vehicle.
