REQUIRED QUALIFICATIONS:
- Graduate of an accredited nursing school.
- Must possess a current license to practice nursing in the state of practice.
- Minimum of two (2) year of experience in general nursing practice, hospice experience and/or palliative, end of life care preferred.
- Supervisory and good communication skills.
- Ability to work well with others, function well under pressure, and function independently.
- Must comply with the standards outlined in the Nurse Practice
- Top notch customer service skills
PREFERRED QUALIFICATIONS:
- Minimum of one year hospice case management experience
- Previous long term care experience
EDUCATIONAL AND TRAINING REQUIREMENTS:
Associate degree in science of nursing
Obtain state required CEU's
CPR certification
State drivers license

KNOWLEDGE AND COMPETENCY:
Must demonstrate basic knowledge of the nurse liaison job description.
Must have the ability to follow instruction from supervisor and other management staff.
Ability to communicate effectively with managers, patients, families and coworkers.
Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patients care.
JOB RESPONSIBILITIES (includes but not limited to the following):
- Develops a hospice presence within hospitals, LTC and SNF facilities, and physician offices by attending and facilitating rounds and care conferences when possible, conferring with providers, speaking at in-service meetings, attending committee meetings, and participating in new staff
- Develops, educates, implements, and serves as a consultant to hospital and community healthcare professionals concerning hospice programs, palliative care, and pain management and about hospice Medicare/Medicaid/commercial reimbursements.
- Act as a liaison and advocate for hospice personnel, facility staff, families, patients and referral sources.
- Ensures the hospice team is in good communication with facilities.
- Helps determine if staff is the right fit in facilities.
- Works with physicians, nurses, social workers, and other medical professionals to assure hospice is delivering the desired expectations.
- Teaches staff customer service when dealing with referral sources.
- Works in collaboration with the HCC to educate and support hospice referral sources.
- Works in accordance with the Branch Administrator to provide unified training. Performs all other duties as assigned.
PHYSICAL AND ENVIRONMENTAL REQUIREMENTS:
Must be able to complete all physical demands of the job, which may include, but not limited to: the ability to lift up to 50 pounds, carry supplies, and read normal type written print. Have corrected vision, hearing within normal range, and have manual dexterity with normal range of motion of all extremities. May be requested to work weekends, holidays, and occasional overtime. Must be able to work alone without usual support systems immediately available in acute care settings.
