POSITION SUMMARY: Under the general supervision of the Wellness Court Manager, the Diversion Case Worker will be responsible for an ongoing case load of Diversion/Wellness participants to include youth, adults, and/or families. This position will aim to reduce offending behavior and recidivism that is directly connected to problematic drug and alcohol use and work with individuals referred to the Healing to Wellness Court to assess their current and ongoing needs, coordinate services and activities to promote wellbriety. Responsibilities include but not limited to case management and the direct supervision of clients to ensure program compliance; screening and eligibility intake assessments to determine and analyze client substance abuse issues; collaboration with the community, and continued support, development, and implementation of Tribal Court Diversion Programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
- Provide case management and wellness program services to both adults and youth; ensuring that program guidelines and activities are age appropriate, accessible, and tailored to their individual needs.
- Complete intake packets with individuals applying for program services. This includes gathering all necessary forms and documents to ensure applications are complete.
- Develop and maintain participant case files.
- Directly supervise and intensively monitor client case management.
- Perform frequent and random drug testing and home visits on a regular basis.
- Monitor, initiate, coordinate, and refer clients for drug testing and inpatient and outpatient treatment options.
- Provide transportation and accompany clients to appointments and court appearances, appointments or as otherwise directed or required.
- Provide information on outside agency services, requirements, and procedures to clients to make appropriate referrals.
- Act as an advocate for clients in interactions with other service entities.
- Facilitate program support and group activities.
- Provide one on one and group mentoring.
- Maintain cooperative relationships with treatment agencies, community organizations, probation departments, Tribal departments, and other agencies.
- Gather and compile quarterly reports on participant activities and services.
- Provide program progress report data to your supervisor.
- Attend and participate in meetings, conferences, and committees to provide background information on the Wellness Program.
- Coordinating activities, trainings, or community events for youth, adults, and/or families.
- Oversee and supervise community service activities, ensuring program participants meet their obligations while adhering to established guidelines and promoting personal growth and community engagement.
- Assist in interviewing individuals and family members to compile information on social, educational, criminal, institutional, or drug history.
- Identify the strengths and needs of the client, identify problems and the capacity for improvement and review and revise the progress of case plans based on personal observations and data collection.
- Administer risk and needs assessment to clients
- Attend Court hearings and prepare written and verbal reports noting the clients' progress and ability to make improvements as outlined in case plans to Tribal and State Court.
- File reports with the Tribal Court on the status of clients as directed by the Judge and/or make case recommendations upon the request of Court or Prosecution.
- Problem solving activities including action planning, identifying resources, goals, and timelines to resolve issues, etc.
- Perform HWC administrative and clerical duties (e.g. answering phones, case file maintenance, document creation, making copies, scanning documents, etc).
- Attend and participate in conferences, meetings and committees as the Bear River Wellness Court representative, travel as necessary.
- Other duties may be assigned.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
- Prefer experience in related fields such as holistic/cultural rehabilitation, family services, children services, mental and developmental disability services, substance abuse programs, juvenile/adult corrections programs, or other closely related field.
- Prefer knowledge of facilities or agencies that provide services to Native populations, socio economic conditions and trends, and the principles of individual and group behavior.
EDUCATION/EXPERIENCE:
- Must be eighteen (18) years of age or older.
- Must have two (2) years of direct experience with Behavioral Science, Social Science, Criminal Justice, or closely related field.
CERTIFICATES, LICENSES, REGISTRATIONS:
- Shall possess a valid driver's license and be insurable by agencies insurance.
- Must provide or obtain CPR & First Aide certification within three (3) months of hire.
COMPUTER OPERATIONS:
- Intermediate personal computer skills, including electronic mail, routine database activity, word processing, spreadsheet, graphics, etc.
LANGUAGE SKILLS:
- Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
MATHEMATICAL SKILLS:
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY:
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
OTHER SKILLS AND ABILITIES:
- Ability to operate standard office equipment (copier, fax, etc.).
- Must be able to work with American Indian people and be sensitive to their culture.
- Comply with all safety policies, practices and procedures. Report all unsafe activities to supervisor and/or Human Resources.
- Participate in proactive team efforts of Bear River Band of the Rohnerville Rancheria.
- Provide leadership to others through example and sharing of knowledge/skill.
- Must be able to work all shifts, weekends, holidays & special events as needed.
- Basic understanding of case management.
- Ability to communicate effectively, maintain active case files for clients, and make informed recommendations on client’s wellbeing.
- Prefer knowledge of facilities or agencies that provide services to Native populations, socio economic conditions and trends, and the principles of individual and group behavior.
- Able to travel locally and out of the area for trainings and certifications.
- Must accept confidentiality as a strict condition of employment.
- Ability to write reports from available data to determine client needs, identify problems and initiate action plans.
