Position Description: Support prescribers and nurses in collecting, organizing, inputting into, or pulling client information, including keying medication information into the Medication Log, retrieving, scanning, follow-up on and importing labs, documenting prescription orders, etc. Resolve requests processed through the Medical In Box, including medication appointment scheduling or changes, Pharmacy requests, responding to client questions and/or concerns related to medications and facilitating requests for medication refills or changes as directed by the prescribers, etc. Facilitate the processing of client prescriptions as directed by the prescribers, including calling them in to the pharmacies and confirming refill and dosage status for accuracy. Corresponding with nursing staff, therapists, and other agencies or providers as appropriate when medication changes are made. Assist in the nurses with bubble packing, packaging, checking, and charting client medications. Assist in arranging for and documenting the delivery of sample medications distributed to clients. Help to maintain an organized sample closet. Maintain an automated schedule, reflecting the FTE availability within Center hours, in order to be consistently available and visible to team members, Center staff, and clients. Keep accurate time sheet records, recording all time worked with the appropriate work code and time duration. Obtain approval from immediate supervisor on any and all leave taken Document billable and secondary service by 30 minutes past closing of the next business day. Field calls not processed through the Medical In Box, order medical supplies, insure the med team has an adequate supply of needed paper forms. Assist with patient assist applications and prior authorizations. Back up nursing staff as appropriate and needed. Attend Center meetings and regular supervision with supervisor, and perform other duties as appropriately assigned by the supervisor. Maintain prescriber schedules; preparing the schedules daily; and printing the med list, noting PCP and labs and releases for PCP; Acting as a liaison with medical staff and clients regarding medication referrals, appointments, prior authorizations, etc.

May provide case management services or Non face-to-face duties, including but not limited to: Coordinating, Assessing, Linking, & Monitoring through face to face, telephone, or written communication with the client, or applicable family member or service agency staff with direct involvement in providing or assuring the client obtains necessary access to medical services. Monitoring the prescribed medication regimen with the client or other applicable individual or agency staff, on behalf of the client, to ensure access to needed medication management services. Time spent on the phone, with a specific client or other applicable individual or agency on behalf of a specific client, for the purpose of monitoring & reviewing the client’s medication & medication regimen, providing appropriate information regarding the medication regimen and resolving medication problems.

OTHER SKILLS, ABILITIES, AND EXPECTATIONS:

  • Knowledge of the practice of the clinical nursing specialty
  • Knowledge of confidentiality and ability to maintain strict compliance with confidentiality of all client and Center sensitive information.
  • Effective oral and written communication skills.
  • Knowledge of community resources available to the mentally disabled.
  • Teaming skills for effective interoffice relations and for working with others in client treatment.
  • Computer literacy and skills for operation of automated medical records, email, scheduling, etc. systems.
  • Timeliness with schedule including arrival to work, whereabouts, attendance at meetings, and client appointments.
  • Document exact and "to the minute" for billings and time sheet entries.
  • Positive attitude related to job expectations, supervision, Center policies and procedures, etc.
  • Ability to behave ethically and to understand and maintain appropriate boundaries with clients and staff.
  • Flexible hours to meet client and/or Center needs.
  • Conduct expected tasks in a safe manner, uphold Center safety policies, and report perceived safety issues appropriately

Job Requirements: A minimum of two years of education beyond high school with medical technology training, experience preferred. Must be able to pass a criminal background screening. Time will be spent in both Logan and Brigham City facilities.

Position Location: Both Logan and Brigham City, Utah

Position Salary: $16.74/hr - $18.83/hr based on education and experience

Position Hours: Mon - Fri 40 hours/week

May be offered additional pay if hired for proficiency with Spanish language. Excellent Benefits. EOE.
